How do I find and execute a revocation document on my sister who is mentally unstable to be poa for my mother? Its an emergency!

It is not entirely clear what you are asking but if it is your Mother's Power of Attorney, only she can revoke it. She can revoke it as long as she is mentally competent and does so in writing. So particular form is usually required but LegalZoom has template revocation of POA that can be utilized. If your mother is not mentally competent and it's an emergency, you might need to file an Emergency TRO and Injunction along with a Guardianship Petition and/or notify your local or State equivalent of the Department of Social Services and/or Department of Aging.
