A Request of Extension of Time to File a Statement of Use ("Extension Request") is a sworn statement signed by the owner (or person authorized to sign on behalf of the owner), that states the applicant still intends to use the mark in commerce, but needs additional time to do so and prove it by filing a Statement of Use and specimen.
If granted, the Extension Request gives the applicant 6 months to either:

Up to five (5) extensions of 6 months each may be requested. This gives Intent-to-Use applicants a total of 36 months (including the initial 6-month period) from the mailing date of their Notice of Allowance (NOA) to file their Statements of Use. If, after all extension periods have expired, no Statement of Use has been filed, the USPTO will deem the application abandoned, and will not register the mark. To view the mailing date of your Notice of Allowance, visit http://tsdr.uspto.gov/ and enter the USPTO serial number for your trademark.

If you do not file a Statement of Use or an Extension Request within 6 months of the issuance of your Notice of Allowance, the USPTO will consider your application abandoned.
