What's an artwork release?
Getting clearance to use artwork in a movie or other filmed story is an important element in an already collaborative process. An artwork release allows a filmmaker the use of a specific work of art, or reproductions of that art, while protecting the owner's rights. This includes the terms of how the artwork will be used, and payment for its use.
Here's the info you'll need to have handy to complete your doc:

Know who this doc is going to and have the individual or business name and contact info ready.

Decide if the owner of the art will be paid a fee, and if so, how much.
